Summit Health custom developed a home test screening for Lipid Profile (Total Cholesterol, HDL, LDL, Triglycerides) and Glucose. The Home Test is mailed to the employee’s home or office, where the employee self-administers the test by placing blood on a specially treated paper the size of a credit card. The employee then mails the specimen in a self addressed and pre-paid envelope directly to our lab for processing and reporting. The entire process is safe and easy, and the employee may call an 800 number at any time for more information and support. After processing the sample, our lab mails the test results and interpretation back to the employee. The laboratory then sends Summit Health the results in an electronic format. Summit Health then uses this electronic report to develop an aggregate report for the client and also has individual results that can be used for follow-up.
Everything is conveniently included in the Home Test kit, including forms and instructions, finger stick supplies, and a preaddressed, prepaid mailing envelope for the blood sample.
Below is a step-by-step process for the Home Test kit:
- Employees are informed of the Home Test kit via a marketing campaign through memos, emails, etc.
- Employees order a Home Test kit via a web site or phone call. Home test product delivery information is collected and the order is processed through to fulfillment including any required prescriptive approvals.
- The Home Test Kit is shipped from our stock; kit delivery is expedited via USPS 1st class shipping.
- The employee receives the kit and follows carefully developed instructions to collect the sample and package it for prepaid shipment to one of our CLIA certified lab network partners for analysis. Each test entails collecting a small amount of blood, urine or other sample that can be safely preserved and transported by mail to the partner lab.
- A toll-free number is available for employees to call with questions.
- The employee then drops the postage paid return package in the mail.
- The CLIA certified lab then analyzes the coded, anonymous sample and uploads the result via the secure lab manager interface. All results are automatically reviewed by medical staff; sensitive outcomes are flagged for personal attention.
- The employee is notified by email that their test results are ready for review online. All data is stored for future access by the employee in full HIPAA compliance and is integrated with the on-site biometric screening data. The employee logs in to their account to review the results securely and privately. Interventions are then promoted if applicable and users at risk are prompted to follow up with their doctor.
