Summit Health provides three HIPAA-compliant reports to clients following immunization and vaccination programs. There are no additional fees for the reports:

      1. Aggregate participation (Vaccinations/Immunizations)
          a. By client
          b. By site

      2. List of participants by site

      3. Insurance claims report and direct bills (for health plans)

      4. Sales report on client sign-up (for health plans)

      5. Participation satisfaction survey report
          a. Employee
          b. Site Coordinator

Reports are available within 5 business days following clinics.
