Summit Health works with clients to develop customized informed consent forms to collect all needed participant data. Participants’ biometric information is collected and recorded by nurses on our Scantron informed consent forms during screenings.

Completed informed consent forms are returned to our corporate headquarters where they are scanned into a relational database with a Scantron system that reads information using optical character recognition (OCR) and optical mark reading (OMR). This automated approach reduces human error associated with data entry. The system also provides field level validation by checking the upper and lower bounds (for example, total cholesterol should be between 100 and 500). If a field exceeds the upper or lower bounds, the data entry staff researches the error. Information from the informed consent form (employee name, address, employee number) and biometric information are loaded into the Summit Health secure relational database.

Summit Health works with the Information Technology department of our customers to define data transmission and integration. Our Information Technology team of twelve staff has met all of our customer’s requirements and requests.

Summit Health works closely with third party organizations in a seamless manner. Summit Health is able to do this based on our large service offering, state-of-the-art Information Technology Systems, and our single-point-of-contact program.

Summit Health can upload data into health assessment, health coaching, health and wellness, or disease management platforms. The most popular method with our client is a secure file transfer protocol (FTP). The Summit Health program manager assigned to each account ensures data uploads are successfully completed.

Summit Health provides four levels of HIPAA-compliant reporting. There are no additional fees for these reports:

  1. Aggregate Screening Results Report. Valuable to both the client and its health plan provider, the Aggregate Report presents biometric data stratified by risk category for each site included in the client’s screening program, and ranks the client’s sites for each test result, as shown in the sample excerpted below. This report also provides year-to-year comparisons for up to five years.

  2. Individual Biometric Record e-File. Available only to the client’s health plan or other approved HIPAA-compliant third-party administrator, this electronic file contains complete individual biometric records with each participant’s name, contact information, health plan number, and questionnaire fields from his or her informed consent form, as shown in the example below. The Individual Biometric Record e-File includes the Aggregate Screening Results Report described above.

  3. Site Screening Report. In addition to the two reports mentioned above, Summit Health provides a report that shows the number of screenings conducted for the client at each site and in total.

  4. Client Satisfaction Survey Report. The Satisfaction Survey Report is an aggregate report of client coordinator or participant satisfaction ratings. Summit Health provides satisfaction surveys for site coordinators, and can provide satisfaction surveys for health screening participants as an option. The report aggregates results by site and in total for each client.
